Life Insurance Plans Basic Life and AD&D Plan
The Center provides Basic Life and Accidental Death & Dismemberment insurance effective on the first day of eligibility in the following coverage amounts:

• Basic Life — two times annual salary
• Accidental Death — two times annual salary
• Accidental Dismemberment — partial benefits for loss of limbs of eyesight. Refer to Certificate of Coverage for a schedule of benefits.

Optional and Dependent Life Plans
The Optional and Dependent Life Insurance Plan lets you purchase additional term life insurance at competitive group insurance rates. You may purchase insurance at the following coverage levels (guaranteed issue amounts are available if elected during the first 30 days of eligibility):

• Employee — From $10,000 to up to $1,000,000 or eight times annual salary, whichever is less. Guaranteed issue amount: $200,000.
• Spouse/Domestic Partner — From $10,000 to up to $250,000 or the amount of your optional insurance, whichever is less. Guaranteed issue amount: $10,000.
• Dependent Children — You can elect between $2,000, $5,000 or $10,000 in coverage. This coverage must be elected during your first 30 days of eligibility or 30 days of the child's birth or placement in your home for adoption.

You must have employee optional coverage to elect optional coverage for your spouse/domestic partner or dependent children. Elections for coverage beyond the guaranteed issue amounts will be subject to evidence of insurability. Contact Human Resources for application forms and procedures.
